Dog and Cat Boarding

Animal Doctor Blue Hills boarding offers spacious, clean kennel areas that are indoor and climate controlled. Dogs and cats are housed in separate sections of the hospital to decrease stress. Baffles are used to decrease noise and provide a peaceful stay. Kennels are fabricated from aluminum and plastic to be as safe as possible. Every day all pets receive fresh linen bedding, along with food and water matching your schedule. We feed dogs Hill’s Science Diet Sensitive Stomach food or you are welcome to bring your own. Dogs are walked three times daily. Kennels are ground level and six feet high, measuring 3’ x 5’ or 3’ x 7’. Your pets can be housed together or separately per your request. Cat condos offer interactive walk through areas and hiding spots to make them feel safe.

We are happy to provide special needs boarding. Every day our trained kennel technicians keep detailed notes about your pet’s status and quickly inform one of our veterinarians if anything is out of order. We are also happy to provide medical services while you are out of town. If you plan to board over the holidays we recommend calling us as soon as possible, so that we can let you know if we have spaces available. We do require owners to come in person and put down a boarding deposit to reserve a space over the holidays.

ANIMAL DOCTOR BOARDING AGREEMENT

1. Owner/Guardian: I represent that I am the owner and/or authorized guardian of my pet and I am fully authorized to enter this agreement.

2. Refusal of Service (pet illness, aggression, etc.): I understand that Animal Doctor Blue Hills reserves the right to deny admittance to any pet for any reason, at any time. I understand that this is for the safety of my pet and the staff. (If your pet has already been dropped off, we will contact you as soon as possible to have you return to collect your pet.)

3. Personal Property: I agree that ADBH shall not be responsible or liable for any lost, stolen, or damaged personal property belonging either to my dog or me.

4. Reservations: I understand that confirmed reservations are required for boarding services at ADBH. I understand that there is a half deposit for some Peak/Holiday periods that is non-refundable unless cancelled a minimum of 14 days prior (some extenuating circumstances will be considered).

5. Check-in & Check-out Times: I understand that I may drop-off or pick-up my pet(s) from ADBH at any time during regular business hours, Monday through Friday 7:30am to 6pm and Saturday 8am until noon. I understand that boarding is to be charged per night and that I will be responsible for payment in full at time of pick-up.

6. Pet Requirements: I represent that my pet meets all of the following requirements: (1) is four months of age or older, (2) is current on his/her required vaccinations (Dogs- Rabies, DA2PP, Bordetella; Cats-Rabies, FVRCP) (3) is on a monthly flea and tick preventative medication, (4) has been in good health for the last 30 days prior to check-in, (5) my dog will enter and exit the facility on a leash, and (8) my cat will enter and exit the facility in a cat carrier.

If vaccinations are not current or if proof of vaccinations is not provided, I give permission for Animal Doctor Blue Hills to update vaccinations in accordance with the above policy.

7. Fleas and Ticks: I agree that if any fleas or ticks are discovered on my pet during check-in or at any other time while my pet is receiving services at ADBH, that ADBH may administer any treatments or preventions deemed necessary at my expense.

8. Health & Veterinarian Care: To the best of my knowledge, I represent that my pet has not had any contagious illnesses of any kind for 30 days prior to check-in. One of the advantages of boarding your pet at Animal Doctor Blue Hills is that veterinary attention is readily available should the need arise. If your pet(s) become ill, we will call the emergency number(s) given above regarding your pet’s symptoms, treatment options, and estimate of additional costs.

However, if no one can be reached, one of three actions listed below will be taken should your pet(s) require treatment to relieve immediate discomfort or to resolve an important medical condition. (Please be aware that should your boarding pet become ill after business hours or over the weekend, attempts will be made to contact the on-call doctor and additional emergency fees may be incurred at your expense.)

9. Veterinarian Liability: I agree that I am assuming all risk of illness, disease, harm or otherwise to my pet by allowing my pet to participate in services at ADBH. The staff at ADBH will do everything possible to keep your pet happy and healthy; however, please be aware of the risks in boarding your pet(s) at any facility. These risks include, but are not limited to injuries, infections, stress, diarrhea, or vomiting.

*The CPR/DNR consent below is to be read and completed by our “Special Needs” boarders and “Medical Care/Hospitalized” patients only. Owners with pets for “General” boarding may also complete this section if so desired.*

Consent for CPR or DNR: In the case of a medical emergency and your pet were to suffer cardiac arrest and/or pulmonary arrest (heart or breathing stops), do you authorize us to provide life-saving measures (ie. cardiopulmonary resuscitation, etc)? Costs of these services can be between $250-500 and are NOT included in the boarding estimate provided. All efforts will be made to contact you as soon as possible to inform you of the situation and given options on how to further proceed. *

CPR- I authorize appropriate life saving measures. I understand that the cost of services may exceed this estimate.

DNR- I do NOT wish for life saving measures to be employed. I am electing “Do Not Resuscitate” status for my pet.
